Com Hab is available to individuals of any age who have been approved for OPWDD services and have an active Life Plan in place.
To receive OPWDD services, a person must have a qualifying developmental disability — such as autism, an intellectual disability, cerebral palsy, or another neurological condition — that began before age 22 and significantly impacts their daily functioning.
Once eligibility is established, Com Hab can be requested through your care manager as part of your loved one's overall service plan. The care manager submits the request to OPWDD, and if approved, services can begin.
Com Hab serves children, teenagers, and adults, with no upper age limit. Some people need support building basic routines at home. Others are working toward greater independence in the community. What matters isn’t the diagnosis someone has, it's whether dedicated, one-on-one support could help them build the skills they need to live more fully and independently.
