How do I get OPWDD services?

OPWDD services can provide life-changing support for families managing a loved one’s disability. Enrolling in services is a multi-step process that can look different for each individual’s circumstances. Our team can walk you through each of the many steps based on your unique circumstances.

To apply for OPWDD services, an individual must first receive OPWDD eligibility – a determination of developmental disability from OPWDD.

In order to apply, one must have: 

  • A diagnosed developmental disability (such as an intellectual disability, autism spectrum disorder, cerebral palsy, epilepsy, neurological impairments, or familial dysautonomia)
  • Significantly impaired functioning as measured by an adaptive behavior assessment which looks at areas of functioning such as communication, learning, mobility, self-care, capacity for independent living, and economic self-sufficiency (for adults)
  • Proof of disability before age 22
  • A New York State address

Once OPWDD eligibility is received, you’ll be connected with a Care Coordination Organization (CCO) if you haven’t yet been assigned one. They’ll provide you with a care manager if you have Medicaid. If you don’t yet have Medicaid, you’ll work with a waiver specialist who will help you get Medicaid. 

Once that’s done, you can apply for OPWDD waiver services. After being approved, you’ll enroll in the services that will best benefit you and begin receiving support.
