What kind of documents or evaluations do I need to get OPWDD eligibility?

The intake process can differ based on your child’s specific diagnosis, needs, or circumstances. 

Typically, these are the documents that will be requested: 

• Your child’s birth Certificate
• A copy of your child’s social security card (front and back)
• A copy of your child’s insurance card (front and back)
• A psychological evaluation (includes IQ testing, adaptive behavior composite, and autism testing, if applicable) and psycho-social evaluation
• A medical form which must include date of exam, diagnosis, signature, and stamp from clinician
• An Individualized Education Program (IEP) if applicable
