What does a care manager do?

Care managers play a crucial role in securing and managing OPWDD support for individuals with disabilities.

Once OPWDD eligibility is confirmed, the care manager creates a Life Plan document – a crucial document for accessing services. It outlines needs and strengths, and includes a service plan that lists the supports the individual will receive. This document needs to be accurate and updated regularly, and the care manager oversees each step.

The care manager regularly checks in with families, follows up with providers, tracks progress, and updates records. When something changes, the care manager adjusts the plan. 

For example, if a child begins struggling with communication at school, the care manager can help the family request additional support or modify the existing plan. If a parent notices new behaviors that are concerning, the care manager will help them figure out which services can address that concern.

A reliable care manager helps families avoid missed deadlines, lost paperwork, or long gaps in services. This is especially important for parents juggling multiple responsibilities, medical appointments, or several children with different needs. Good coordination saves time, reduces stress, and keeps support consistent.
