The Life Plan is an evolving document that plays a central role in an individual's OPWDD services. It summarizes an individual's capabilities, goals, and needs, and serves as their roadmap for services and supports — ensuring everything is personalized fo the individual and aligned with their goals, preferences, and needs.
Who participates in a Life Plan meeting?
The Life Plan is developed by the care manager at a Life Plan meeting, in collaboration with the individual's full care team — including family members, advocates, service providers, clinicians, and anyone else the individual chooses to involve. Most importantly, if possible, it's built around input from the individual themselves.
Because many OPWDD services are tied to the Life Plan, it's important for individuals and families to review it carefully and ensure it accurately reflects their needs and goals.
How often is an OPWDD Life Plan updated?
The Life Plan is reviewed and updated regularly — at least annually, or whenever there are significant changes in the individual's needs, goals, or circumstances.
