How do I get Com Hab?

Community Habilitation (Com Hab) is an OPWDD waiver service. To receive Com Hab, an individual must first be granted OPWDD eligibility, which is provided to individuals with a developmental disability that impacts their daily functioning, via a comprehensive application process.

Then, your care manager can request Com Hab service, and if approved, you’ll be connected with trained providers who can support the individual in building practical life skills at home and in the community.
